Didion Golf Sponsorship Form

A Gravity Forms-based sponsorship submission form for Didion's annual golf outing, integrated with Zapier, Google Sheets, and PayPal. As of the January 2026 sync, the form is partially functional with two open blockers before it can go live.

Form Design

Diana consolidated all sponsorship tiers and the golf ball toss add-on into a single form, replacing an earlier multi-form approach. Credit card data is intentionally excluded from the Google Sheets export; all other fields are captured.


Open Blockers

1. Email Notifications Not Triggering

Form submissions are reaching the Google Sheet via Zapier, but Diana is not receiving email notifications to her inbox. The Zapier Zap (Gravity Forms Golf Outing Sponsorship → Google Sheets) was confirmed live and passing test entries. The email notification issue is separate — likely a Gravity Forms notification configuration problem rather than a Zapier issue.

Owner: Heshaq (developer) to investigate and fix.
Melissa's action: Follow up with Heshaq directly.

2. PayPal Sandbox Testing

The PayPal payment gateway connection has not been verified. Diana flagged that sandbox/test mode needs to be confirmed before launch — she needs to know what credentials or configuration Heshaq requires to enable a test transaction.

Owner: Heshaq to clarify sandbox requirements.
Diana's action: Confirm with Heshaq what is needed from her side (e.g., PayPal sandbox account access).


Sponsorship Landing Page Redesign

Diana also requested a redesign of the golf sponsorship landing page (currently structured around individual "click this button" sponsorship tiers, now outdated given the consolidated form).

Scope TBD: Melissa needs to confirm with Mark whether a single-page redesign falls under the existing retainer or requires a separate proposal. A previously sent proposal covering multi-form updates (~$2,000) is likely moot given the single-form consolidation.
